The gut problem
Semaglutide is a peptide. An injection skips the digestive system and releases the drug slowly from under the skin. A pill has to survive the gut and cross the stomach lining, and much of an oral dose still breaks down. Oral semaglutide, approved in 2019 for type 2 diabetes, uses a helper molecule to get across, and is taken on an empty stomach with a 30 minute wait before eating.
The dose math
A 2026 review cited in a manufacturer's press release says that when less than 1% of an oral dose is absorbed, the dose may need to be 10 to 100 times higher. Its example: 1 mg a day by injection could mean about 100 mg a day by mouth. Another example in the release shows a 20 mg oral dose giving exposure similar to a 0.1 mg injected dose.
